How to Transfer a Patient
- Fill out the transfer form in HMED
- Get an accepting doctor if this is an ED to ED transfer.
- If another service is involved and it is applicable, have them contact
their counterparts at the other hospital and get an accepting in-patient
attending.
- Add these names to the form and print it. Have the your ED attending sign the form.
- Give the form to the clerk up front and if we are arranging the
ambulance, tell them what kind of ambulance you need:
- BLS
- ALS
- ALS with ventilator and/or IV pumps
- Tell the clerk if you will need a CD of radiology images. This can take
up to an hour, so plan ahead.
- Call the AOD if after 4 pm; they need to approve the transfer. Between
9am and 4pm, talk to one of our ED administrators, they will put you in
touch with Chris Constantino's office (41638).
Bellevue
Call ED 212-562-3015 and ask for the ED attending. They might give you some
rigmarole about needing to call Dr. Haiman; tell them that is only for inpatient
transfers and your transfer is ED to ED.
Sinai
If another service involved, get them to give you a name for an accepting
physician
